Enrollment

Enrollment for new students opens August 2, 2022

Please note that the Miami Public Schools will verify that your resident address is within the Boundaries of the Miami Public Schools District. If not, you may apply for an open transfer and, upon approval, attend a traditional setting.

Enrollment packets can be picked up at the locations listed below.


Below are the instructions to complete the new student enrollment process:

  • PreK 3- Kindergarten Students (Traditional or Virtual) need to pick up and complete an enrollment packet of forms from Wilson Early Childhood Center at 308 G NW, 918-542-8419.

  • Grades 1-3 Students (Traditional or Virtual) need to pick up and complete an enrollment packet of forms from Central Office at 2116 Rockdale Blvd, 918-542-8455.

  • Grades 4-6 Students (Traditional or Virtual) need to pick up and complete an enrollment packet of forms from Nichols Upper Elementary at 504 14 NW, 918-542-3309.

  • Grades 7-9 Students (Traditional or Virtual) need to pick up and complete an enrollment packet of forms from Miami Junior High at 2000 East Central, 918-542-5588.

  • Grades 10-12 (Traditional or Virtual) need to pick up and complete an enrollment packet of forms from Miami High School at 2000 East Central, 918-542-4421.

All students must provide:

  • Proof of Residency reflecting Name and Property Address (rent receipt, lease agreement or mortgage document)
  • Utility Bill (Water, Gas or Electric Bill)
  • Student’s birth certificate
  • Copy of at least one parent/guardian current ID
  • Custody Documentation if applicable
  • Proof of immunizations*
  • Students Tribal card or CDIB Card (if applicable) or Parent CDIB Cards

 

*The immunization exemption form can be found at this link: https://oklahoma.gov/health/services/personal-health/immunizations.html  If you complete the exemption form, a copy must be submitted to Lisa Namestka (lnamestka@miamips.net) at Central Office in order to document and submit for approval.

You will also be asked to complete the following forms at the physical location. (They require a physical signature on a printed paper)

  • Home Language
  • Economic Disadvantage
  • McKinney-Vento
  • Migrant
  • Computer Device Contract and Internet Agreement
