Enrollment
Please note that the Miami Public Schools will verify that your resident address is within the boundaries of the Miami Public Schools district. If not, you may apply for an open transfer and, upon approval, attend a traditional setting.
During the school year, enrollment packets can be picked up at the locations listed below.
During June and July, all packets can be picked up and dropped off at Central Office (601 16th Ave. NW).
Below are the instructions to complete the new student enrollment process:
PreK – Kindergarten Students (Traditional) need to pick up and complete an enrollment packet of forms from Wilson Early Childhood Center at 308 G NW, 918-542-8419.
Grades 1-3 Students (Traditional) need to pick up and complete an enrollment packet of forms from Central Office at 601 16th Ave. NW, 918-542-8455.
Grades 4-6 Students (Traditional) need to pick up and complete an enrollment packet of forms from Nichols Upper Elementary at 504 14 NW, 918-542-3309.
Grades 7-8 Students (Traditional) need to pick up and complete an enrollment packet of forms from Miami Junior High at 2000 East Central, 918-542-5588.
Grades 9-12 (Traditional or Blended) need to pick up and complete an enrollment packet of forms from Miami High School at 2000 East Central, 918-542-4421.
- Virtual K-12 need to pick up and complete enrollment packet forms from Miami Innovation Center. 504 Goodrich Blvd, 918-542-976091
All students must provide:
- Proof of Residency reflecting Name and Property Address (rent receipt, lease agreement or mortgage document)
- Utility Bill (Water, Gas or Electric Bill)
- Student’s birth certificate
- Copy of at least one parent/guardian current ID
- Custody Documentation if applicable
- Proof of immunizations*
- Students Tribal card or CDIB Card (if applicable) or Parent CDIB Cards
*The immunization exemption form can be found at this link: https://oklahoma.gov/health/services/personal-health/immunizations.html If you complete the exemption form, a copy must be submitted to Lisa Namestka (lnamestka@miamips.net) at Central Office in order to document and submit for approval.
You will also be asked to complete the following forms at the physical location. (They require a physical signature on a printed paper)

- Migrant
- Computer Device Contract and Internet Agreement
